Comment:

 The issue is back with 2.8.6 and its worse, I fixed it last time by
 reinstalling, it doesn't seem to go away at all now. I have found that if
 I do the following it works on a per user basis...

 1. Reset user x's password
 2. Login as user x
 3. Go to new post
 4. Observe the missing media buttons
 5. Make user x site admin in 'Site Admin' -> 'Options'
 6. Reload/login to user x and observe it works.
 7. Demote user x from site admin.
 8. Reload/login to user x and observe it still works after all this.

 Its like its resetting a cache?
