 Sorry you never got a proper response to this report. There have been no
 further reports of general problems with the application password feature,
 so I'm going to close this off as something that was likely caused by a
 caching issue, incorrect API usage, or something else on this particular
 site that was causing the functionality to break.

 If the problem still exists and you can reliably reproduce it, please
 comment with some more details. Cheers!
