Although it hasn't been updated for a while, as far as I'm aware this
plugin (http://wordpress.org/extend/plugins/taxonomy-metadata/) still works
fine.

At least it is working happily on a site I built a couple of years ago,
which is updated almost daily and which has more than 40,000 records using
multiple taxonomies with meta data.
