Yup, http://codex.wordpress.org/TinyMCE_Custom_Buttons is very useful,  
but Viper's video-quicktags plugin did teach me a few tricks of the  
trade to manage integration with TinyMCE 2 and 3, as well as buttonsnap.

All is (on its way to be) well under the sun.
