 I think I may have the reason:

 Maybe this happens when a notification is generated by a user who is later
 marked as spam by an admin. The notifications should go away when users
 are marked as spam. Also when marked as spam directly in the user list.
 (This is the only place I can stay on top of the hundreds of spammers
 registering every day on my site.)
