 Ok thanks, that sounds reasonable. I'm entirely happy to maintain a fork
 of the .htaccess locally.

 Before I dig around in wpmu-blogs.php, is there a canonical list of
 characters that "should" be allowed in blog names? The system I'm tying
 into allows usernames that match ^[a-z0-9][a-z0-9\+\.\-]+$
