Add New Category Screen

If screen options are set to just show slugs, new items added are displayed
as if all screen options show.
This results in the appearance that the slug has not created correctly.  (It
is showing floating to the right of the slug column)

If you edit a category & return to the add new category screen, screen
settings revert to default. (I personally would be fine with disabling the
screen options on this page.)
