[wp-pro] Searching For WP-Plugin: Login Activity Feed

I'm searching for a plugin that can record/report Login Activity of actual members who've logged in on your site, OR for USERS who have filled out a form on your site.  I want to have this to be viewed on the Homepage. 

For example, when a member logs into the site, the homepage sidebar would showcase a the Name of user online.

Also, if this can be done when a person simply fills out a form with their name, where an activity feed can be showed.


Note:  Not other type of activity needed, just activity of a person Logging into the Wordpress site, or a person filling out a form.

Can someone point me in the right direction of any existing plugins out there?
