When you create an additional user account on an MU subdomain site, the
user account does not work correctly.
For example, assuming a WPMU subdomain installation is intalled at
http://domain.com (blog id 1), and blog id 2 is set up at
http://blog2.domain.com/.
1.Login to a http://blog2.domain.com/ (not blog id 1) as an administrator
of that blog.
2. Dashboard, Users, Add New User, enter username and email of someone
that doesn't already exist in the WPMU installation. Submit the form.
3. Go to email to collect invitation for verification. The email looks
like:

You've been invited to join 'Blog 2' at http://blog2.domain.com/ as a
subscriber.
If you do not want to join this blog please ignore this email. This
invitation will expire in a few days.
Please click the folowing link to activate your user account:
http://domain.com/wp-activate.php?key=4278fce539b1d152
}}}
4. Click the verification link
Problem 1: once the verification link is clicked, I am sent to an
domain.com page, not the blog2.domain.com page. This will be
confusing to the user, as they won't know about domain.com, just
blog2.domain.com.
Problem 2: There is no link to blog2.domain.com - even if I go back to my
email, the confirmation email doesn't mention blog2.domain.com anymore.
5. Invite is now verified.
After verification, this email is sent:

Your new account is setup.

Password: xxxx
http://domain.com/wp-login.php

This isn't helpful, what I should get is a message with a
login link blog2.domain.com.
6. Login to blog2.domain.com as the new user. First view is the
Dashboard, User Profile (presumably because the user account wasn't
correctly set up on blog id 2).
Problem 3: click on Visit Site and an endless redirect occurs.
