Comment:

 On my install it seems to happen on any blog where any MU admin is also a
 user of any level.

 donncha, If you would like access to my clone install then please let me
 know, this should allow you to investigate the issue properly.

 The install I'm upgrading is a replica of a live site to test upgrades
 first before doing it on the main install, thus you will have full access
 where required.
