 Do you have BuddyPress installed? I think this is a problem in that. I
 spent a few hours debugging it and then discovered the
 "wpmu_signup_user_notification_email" filter in
 bp_core_activation_signup_user_notification() in wp-
 content/plugins/buddypress/bp-core/deprecated/bp-core-deprecated.php which
 stops the original MU function working.

 The filter in that function takes two parameters (the MU one did too
 originally I think) which messes things up unfortunately.
