 I think the problem is related to installing the plugin in mu-plugins/
 rather than the plugins folder. The menus appear fine when the plugin is
 in plugins/

 The new "sitewide plugins" code should make using the plugins folder more
 popular and this problem will remain a minor one. Can anyone else confirm
 this?
