 I've tried the patch and then updated it as follows:

 1. Cleaned up the rewrite rules
 2. Removed the call to iis7_delete_rewrite_rules. While the cleanup of the
 IIS config file makes sense for WPSU, it should not be done for WPMU as
 without rewrite rules WMPU stops working.
 2. Modified the condition logic in options-permalinks.php.

 The updated patch is attached.
