Comment:

 One thing could be the PHP memory limit... when it crunches, it resizes
 those images for use as different sizes (ie thumbnail,
 small,medium,original).  There could be not enough memory to do that,
 which would explain the post insert problem as well.  Do you know the PHP
 memory limit on your server, and how muny plugins do you have activated?
 (including mu-plugins)
