 Ah, that's PHP's error logging. Something got printed to the browser in
 wp-db.php, line 135, but line 57 of wp-comments-post.php tried to issue a
 header().

 Unfortunately you can't tie a URL into it I think, but it has to be
 someone or something leaving a comment - wp-comments-post.php is never
 included by anything else.
 Check the timestampt too in your access log. I bet you'll see a call to
 that file at that exact second.
