 In index.install.php, lines 240-241, there should be a default of
 checked="checked" on one of the selections.

 These are the radio buttons that designate a subdomain/subdirectory
 install, and specify vhost yes/no.

 Whichever one is checked by default doesn't really matter, as long a one
 is checked. I would think subdomain would be the logical default, mainly
 because that's the recommended installation type.

 Either that, or leave no default checked, and throw an error before
 proceeding if neither one is selected.
