With no follow-up, I'm inclined to close this as 'worksforme'. Please feel
free to comment if you feel this is still an issue that needs to be
addressed.
However, based on the description, it sounds like either a situation that
WordPress doesn't address (replacing image URLs when an image is replaced)
or a caching issue, as even if you replace an image with one of the same
name, your browser may still deliver the previous image temporarily, due
to browser caching.
