Comment:

 Support for `optgroup` would also needed to be added to the JavaScript
 control template, I think, not just the PHP template.

 I also wonder about the `$optgroup` property in [attachment:"47692.diff"].
 What happens if a multidimensional array is used for `$choices` but
 `$optgroup` is `false`, or vice versa?
