 @westonruter can quickly fix 1 and 3, but I'm wondering about 2. Shouldn't
 we change the `widget` method to use `$this->name` instead to keep
 consistency? In this case, it looks like we're repeating the same string 3
 times (`__construct`, `widget`, `form`) which feels kind of weird.
