 Initial WIP patch, This doesn't touch the fact that the url's may not be
 unique (page and post with the same slug, Attachment with the same slug as
 a 2nd level page, 2nd level page with a same slug as an attachment, etc).
 Seems to work in all cases i throw at it.

 The priority order for matching items appears to be: `Posts > Attachments
 > Pages`
