Another idea ... if you haven't, look at wptouch. The default themes are
decent. You can create custom child themes if you want / have time. Create
a separate menu in the WP menu editor, with just the content you need for
this aspect. Set wptouch to use that menu. Define a different front page in
wptouch, if you want, maybe make the form the front page. Outside of
whatever time you need to recreate the form, the steps I outlined above are
mabye an hour, and you have a ipad friendly "theme".
