Using the main/default WYSIWYG editor is something we are considering
doing, but I would need to figure out how to move it down to the
bottom of the interface so as to not confuse the client when they are
entering data (I did find a tutorial online telling me how to do
that). That still wouldn't really completely solve the underlying
issue that I ran into.

What if I have the main/default WYSIWYG editor and 2 custom meta boxes
with WYSIWYG editors and the WP Media Library *Upload/Insert* icons? I
would still encounter this issue if I tried to upload an image from
one of the custom meta boxes (I believe ... I haven't tested to
confirm this).

I'm wondering if I have something set up wrong or if it is something
in the WP core that is causing some sort of link to be missed when an
image is uploaded from a custom meta box.

I was going to dig into the code to see what is rendering that *Insert
into Post* button in the media library lightbox, but I have not had a
chance yet.
