I've read your article and I think it's by far the most useful thing one can
do about the matter. Doing a large scale renaming isn't going to help nearly
as much.

Regarding themes, I think that's a non issue too: most themes do use
the_excerpt() which will output the manual excerpt if it exists. So it's
mainly up to the users to find the time to write them.
