[wp-hackers] Comments moderated, but yet they appear

The commentor will be able to see their comments, And it *may* be that  
Logged in administrators can see unmoderated comments too

Have you checked that when viewing the comments page in a browser that is  
NOT logged in in any form (Clear the cookies too) that you CAN see the  
unmoderated comments?
