I think a combination of the two could possibly cover enough ground. 
Nominations are sent in, so its a broader spectrum, and then mavens
make the final decision, thereby it not being soley upon them to
decide.  It shouldn't be too difficult for them to do a quick glance
at their recent replies, to determine how often and what quality the
replies were.  That way, it doesn't come across as arbritrary, nor is
it a situation where someone "gamed" as you put it, themselves into a
position.

Being new to this community, and a bit overwhelmed by it at times, as
an outsider, I think #4 is proabably going to be the most important
issue you face.  In the global/online community, it seems quite
difficult to genuinely know intentions, and personalities.  I can say
I've drawn my own conclusions about some I've seen around WP, but it's
hard to nail down.  So those who've really committed to this HAVE to
feel comfortable with the choice.
As for titles, not sure, but you can count me as a WP habitué...
