[wp-docs] Template Hierarchy Diagram

Okay, I've updated the Codex entry with a simplified version of the
Template Hierarchy diagram:
http://codex.wordpress.org/Template_Hierarchy#Visual_Overview

I've retained the link someone added to the "advanced" version on my site.
Let me know if this needs to change. (e.g. I'd be happy to upload the
"advanced" version, too; just let me know the best practice here.)

Also, I screwed up my initial upload, by uploading a new file, instead of
replacing the existing file. How do I delete this one?
