[wp-docs] Reverting changes in the Codex

The above Codex page is no longer useful in its current state. It's
full of messy, insecure code (eg. no nonces) and does not conform to
general WP coding guidelines. I propose reverting it to revision 54048
[1] where it had good, clear documentation for creating Options pages
and where the code used lots of built-in methods for managing options.

I didn't want to revert it without asking if this was a good idea or
not. Opinions please!
