[wp-docs] New Codex page on 2.5 plugin/theme migration...

I've made a new Codex page that you may be interested in -- covers the 
highlights of how to migrate plugins and themes from 2.1 to 2.2, 2.3, 
and the upcoming 2.5, with links to earlier articles on how to migrate 
from 1.2 to 1.5, and 2.0 to 2.1.

I also updated previous links on the Plugins Resources and Templates 
pages, as well as Developer Documentation, to point to this article
