[wp-docs] 5-Step Upgrade in Upgrading WordPress

It isn't so much a WP problem, but we've seen issues where the FTP
client doesn't properly overwrite the file when it should. I've
experienced it from time to time. There have been times where despite me
telling it to "Overwrite All" .... it doesn't...and there doesn't seem
to be any rhyme or reason for it.
