[wp-docs] Revising the upgrade instructions

I'm struggling with revising the upgrade instructions for the 2.0 release.

http://codex.wordpress.org/Upgrading_WordPress when 2.0 is official.

Issues to address:
1.  Should the .htaccess file be backed up and what should be done with that
file once the upgrade is completed?
2.  What should be deleted or not deleted in wp-content?  We advise users
not to use FTP to overwrite files so what should be the recommended
approach?
3.  How to address the permission issues regarding the wp-db-backup.php
plugin?
