I second this recommendation. Worst Offenders has been an invaluable tool 
for the blogs I manage.

Another feature I would like, and have been planning on making myself for 
months now but never got around to it: an option to keep the standard 
WordPress blacklist filter active, BEFORE akismet filtering. Blacklisting 
just a handful of spam words (drug names) would dramatically cut down on 
the amount of spam I have to wade through to verify there is no ham.
