 This patch includes code for #496. I created the designs a bit based on
 https://glotpressdotcom.files.wordpress.com/2011/10/glotpress-header-
 ui.png except that I want the header to be able to contain more
 information.

 It does contain breaking changes:
 - Removed action {{{after_hello}}} and replaced with a filter for the menu
 {{{gp_nav_menu_items}}}
 - Different markup for breadcrumbs
 - Markup for breadcrumbs is generated on the getter side instead of setter
 which could break things. As far I tested it it should only be UI breakage
