Comment:

 It seems that in the image src, the URL is:
 https://secure.gravatar.com/avatar/?s=64&d=mm&r=g, which leads to a 404
 error. I guess the correct URL should be:
 https://secure.gravatar.com/avatar/?s=64&d=mm&r=g%202x. It's missing that
 "%202x" at the end.

 Note: Images not showing sitewide for the users who don't have a profile
 photo.
