 Continuing with the style improvements recently done in #7080, it's time
 to add some responsive goodness to the Extended Profiles screen. The
 screen inherits styles set in WP's forms.css which is desktop-first with a
 breakpoint at 782px (max-width).

 In addition to the 782px breakpoint when the radio buttons and checkboxes
 increase in size from 16px x 16px to 25px x 25px in forms.css, I added a
 second breakpoint at 480px for this screen.

 Patch tested on WP/BP Trunk and WP 4.0.11/BP Trunk along with the before
 and after screenshots coming up.
