 Update:

 our server's hard drive failed in June and the whole thing was rebuilt
 with a new hard drive, new os install, new LAMP stack, latest WordPress
 (at the time 3.9.2), latest P2 (current), and latest BuddyPress (current)
 and we are still experiencing thethe same bug.
