 I've put a latest version on Github:
 https://github.com/karmatosed/refreshdefault. This now has the numbered
 table of contents to correct numbers and some small tidying.

 I plan this week to explore removing all inner shadows and
 borders/backgrounds on form elements. However, have to see the impact of
 this. Not all themes declare them so it could look bad. It would make it
 easier for dark/different colour themes though.
