 Sorry for the slow response.  I wish I had better news to report, but I
 haven't made much progress in learning how to debug in Netbeans (got
 sidetracked with other things).  I have xdebug running, but anytime I put
 in a breakpoint it just opens up the wp-blog-header.php file.  Obviously,
 I'm doing something wrong.

 Anyhow, about the only thing I can confirm at this stage is that it is
 definitely happening with all plugins (sans BP) disabled and the
 functions.php file empty.  I'm honestly surprised I'm the only seeing this
 issue, but hey, I guess that's a good thing.
