Comment:

 When I updated my theme from 1.1, I just added the button manually. I
 imagine that most holdovers from 1.1 can do (or have done) the same.

 I'm going to mark this as Future Release so that we can reconsider the
 question as the WP 3.1 admin bar begins to come to fruition.
