 This may be on older entries.
 After you made changes and cleaned the activity content from time since
 etc, This problem occurred on older activity.

 This mainly occurs for those activity for which 'action' field is empty in
 database. I checked it in the template tags and found, you are checking
 the action and if empty populating the  meta. So, That is the reason for
 it.
