Comment:

 I'm getting a very similar error.  My plugin uses both pluggable.php and
 upgrade-functions.php.  If I enable it, when trying to edit or publish a
 page, I get:

 Fatal error: Call to a member function flush_rules() on a non-object in
 /Applications/XAMPP/xamppfiles/htdocs/wordpress-mu/wp-
 admin/includes/mu.php on line 1177

 Disable the plugin it goes away.  Comment out the requires_once lines in
 my plugin it also goes away (but then causes other issues in my code).
