 Actually after working with the standalone WP it turns out the add_rule
 routines are only handling a single Rule and not looping through all the
 entire rulesets that WPMU are required which is causing the 500.14 error.

 The standalone WP 2.8.1 version only has one rule so it never errors out,
 it's also only initiated when someone goes to the permalinks page while I
 noticed that WPMU get's initiated when core_update is set to upgrade
 (which it has been in constant state lately) which probably is the root of
 it erroring out.

 So I'm going to say that this error is due to the upgrade state being
 active and is dependant on the core_update state to be fixed before moving
 further with a proper fix.

 This web.config error is dependant on the status of #1037
