 Typecasting that maybe_unserialize() call was actually causing the blank
 entry bug. By typecasting it as an array it actually decides to add an
 empty first entry when serialized back up. It works fine without it.

 'admin_init' would work - but it would cause a few extra queries on every
 page load. Not sure if it is necessary?
