Comment:

 The custom fields metabox used by posts comes with significant UX and
 performance issues. I don't think we would want to reintroduce those to
 another page. A far better solution is to register form fields using the
 existing hooks on that page.

 If you aren't comfortable writing code, I'd recommend using a Fields
 plugin like Advanced Custom Fields:
