 Tested a bit using Internet Explorer 11 (Windows 10) and noticed there's
 something wrong when the placeholder gets injected, to reproduce:
 - select some text or just place the cursor in the middle of a word
 - press either Shift+Alt+a or Ctrl+k
 as soon as you do this, the cursor is moved out of the selection so the
 toolbar doesn't appear. Seems the placeholder gets injected though (the
 text becomes underlined).
