 Cleanup audio/video shortcodes in the media modal:

 * On the controller's `update`, `replace`, and `close` events, call
 `detach()` on the frame
 * Cleanup the HTML ouput of the Underscore templates.
 * Move some logic from the Underscore template to the `VideoDetails` view
 class.
