 Just wanted to add my two cents.
 I have implemented a menu builder as a add-on for a specific theme
 here is what it looks like

 How it works is: You click on a link that adds a box of  pages,
 categories, links, or a blank text box.

 For example if you want to add a number of pages in a row. You would add
 the pages box and the check off all the pages that you want to add to the
 navigation.
 Help full links such as check all and collapse make it easier to add pages
 if you have a large site.

 If after that you want to add a category you would add the category box
 and do the same.
 You can continue building you navigation that way. By adding boxes of page
 and categories or links or static text.

 The static box helps with the addition of a search form for example.

 Currently there is no way to reorder the boxes by dragging and dropping
 them around. Which could definitely improve the usability.

 Also a way to maximize the box would be great. You can see a whole site
 map without having to scroll in a small box.
