 At the moment, moderation emails are sent when a comment is unapproved or
 removed from the spam. This should not be the case; by changing a
 comment's status the user has taken a moderating action on the comment and
 is aware of it. He doesn't need to be told about it by email.

 Moderation emails should only be sent for new comments.
