[wp-testers] Adding Media Error

Are you running on of my plugins on trunk?
Specifically Add From Server?
If so, You'll probably need to upgrade to a not yet released version :)

I'll have a fiddle and see if I cant make it compatible with both 2.5 and  
2.6
