fixed in a away that it updates the databases of existing sites that
installed with version 2.0 and newer?

I don't see that in the attached dif.  It would be worth adding I think, but
we'd have to make sure the update procedure doesn't blow if the index
already exists.  We've already had comments that say that adding this index
significantly speeds up sites and people complained about the speed of
2.0.4.

Sorry I didn't double check this earlier...
