Also, when 'Linked to Page' is selected for an image, the resulting
attachment page does not validate because the image has no alt
attribute.

Another thing (seeing width="400" in the code got me thinking)... how
is the width of images on attachment pages specified - how will theme
authors change the maximum width of these images to suit their design?
